As a member of the Cobb County Multi-Agency Supportive
Housing Program, Travelers Aid provides case management to families in four transitional
housing units. The five agencies involved (Center for Family Resources, the Cobb YWCA, the Cobb
Community Services Board, the Cobb Human Services Coalition Long Term Family
Support
Team, and Travelers Aid) collectively establish policies and procedures
for case
management
and program operation. Oversight is handled by the
Center for Family Resources.
Travelers Aid is a founding member of the collaborative of service
providers that has launched the Pathways Computer case management database. By bringing
agencies on-line and sharing their client data, the Pathways system enhances
collaborative relationships, prevents duplication of services, extends limited
resources, and increases the impact of the services provided by the member agencies.
